The joint project eMMa is developing digital services for patient-centred eMedication Management as an add-on to the medication plan (AkdA4), which addresses the intrinsic needs of the patient and puts the role of the responsible patient in the foreground. Project description

The aim of the project is to improve drug therapy safety by means of eMedication Management. With eMMa, we are developing, testing and evaluating supplementary value-added services that support eMedication Management in a platform-neutral way.

One of the main challenges in the project is the involvement of the patients themselves. By empowering the patient, deviations from the doctor's prescription are to be avoided and the success of the therapy improved. Technically challenging for a coordinated eMediaction management is also the inclusion of the patient as well as their other health care providers such as hospitals, care facilities and pharmacies. In order to address these challenges, two use cases were developed in which all the work of the project partners is brought together.

  • Use Case 1: Early detection of side effects
    The eMMa consortium is developing interdisciplinary value-added services for the recording of side effects during medication changes by integrating patients' mobile devices.
  • Use Case 2: Promoting acceptance and inoperability
    The eMMa consortium is developing transformation and communication services as an add-on to the medication plan that directly involve patients and provide supplementary information to overcome the sector boundaries between GP, hospital and pharmacy.

The extent to which interdisciplinary eMedication Management is suitable and for which patients it can be successfully applied will be investigated by accompanying quality assurance and evaluation measures.

Results

As a first application, we have made available a web app at mein.klinikplan.de with which patients can keep their own medication plan on the basis of the nationally uniform medication plan. This can be created in preparation for a planned hospital stay, for example, and passed on digitally.
